Mangaluru, Jul 7: Kasturba Medical College Hospital, Attavar, has once again demonstrated its multidisciplinary strength and clinical excellence by successfully managing a complex orthopedic trauma case in a severely obese patient with multiple comorbidities.
A 39-year-old male weighing approximately 200 kg, with a body mass index (BMI) over 53, was referred to KMC Hospital, Attavar, after sustaining bilateral distal tibia and fibula fractures in a road traffic accident. The patient posed an extraordinary clinical challenge due to his extreme obesity, severe obstructive sleep apnea (OSA), chronic hypoxia, and poor pulmonary function — rendering him unable even to lie supine.

Recognizing the complexity and urgency of the case, a coordinated effort was initiated across multiple specialties. Under the guidance of Dr Akshatha from the department of Anesthesiology, the patient was stabilized and optimized in the ICU with critical input from the Pulmonary Medicine team led by Dr Visak Acharya.
Following stabilization, the patient was taken up for bilateral closed reduction and intramedullary nailing of the tibia by the orthopedic team led by Dr Atmananda Hegde, with Dr Chetan Shetty and Dr Keerthan Ranga Nayak U. The anesthesia care was led by Dr Sumesh T Rao, who, along with his team, meticulously navigated the challenges of positioning, difficult venous access, airway management, and neuraxial anesthesia in the setting of morbid obesity. Non-invasive ventilation was used to support breathing throughout the perioperative period.
The case was successfully managed with unwavering support and leadership from medical superintendent Dr Chakrapani and deputy medical superintendent Dr Deepak Madi, whose administrative coordination ensured seamless teamwork and resource mobilization.
The surgery was uneventful, and the patient had a smooth recovery in the ICU. He was shifted to the ward on the second post-operative day and later discharged in stable condition. During follow-up at the outpatient clinic, the patient was found to be recovering well.
