Dubai: Woman regains forehead shape after extended mass removal with Bi-coronal approach

Dubai, Mar 3: A 27-year-old female patient consulted at Zulekha Hospital Dubai’s plastic surgery outpatient clinic for the management of two extra growths on the head and forehead. The patient had a history of trauma on the head three years ago, after which she noticed a swelling on the front side of her head. The swelling gradually but slowly increased in size. A second growth was noted on the forehead a year ago, which was rapidly increasing in size. On experiencing pain in the swollen areas she consulted Dr Chandra Bose, Consultant Plastic Surgery at Zulekha Hospital Dubai.

On clinical evaluation, there were two substantial extra growths. A smaller one in the front side of her head, and a second, larger one on the forehead. There was a bridge of normal tissue between the two. MRI and Ultrasound evaluation of the abnormal growths was done, which showed this was a single mass which was bilobed.

The patient wanted the extra growths to be removed without any scar on the forehead. The surgery was planned. The swollen extension was removed by a Bi-coronal approach through the scalp within the hair line to avoid any scar on the forehead. This approach is technically challenging for a surgeon. The surgery was successfully performed and the extra growth of 7x3x1.5cm in size was removed. The extracted mass was then sent for evaluation by the histopathology lab.

The histopathology department carried out special tests to identify the tumor. Sharing his views Dr Bose said, “A rare variety of tumor called Deep Benign Fibrous Histiocytoma was diagnosed. This variant of Histiocytoma is very uncommon and only very few cases have been reported in the world literature. The treatment of choice of this type of tumor is complete surgical excision. According to the literature there is a 20% recurrence rate, and metastasis (spreading to nearby areas of the body) is very rare.”

The patient has had a full successful recovery with no scars on the forehead. She is currently on regular follow-ups in the plastic surgery outpatient clinic.
