For many women with severe endometriosis, IVF is the most viable route to pregnancy—but the path is rarely straightforward.

A 28-year-old woman with severe endometriosis, no prior surgeries, and a strong desire to conceive was advised to undergo IVF. Initially, everything appeared normal after her egg retrieval procedure. But four days later, she developed a persistent high fever that did not respond to antibiotics. For Dr. Madhuri, the case was a poignant reminder of the emotional stakes in fertility care. “This was a young couple who had walked in full of hope for pregnancy and suddenly faced losing everything—her health, fertility, and the future they had envisioned. The challenge was not just saving a life, but protecting hope,” she reflected.

Months later, the patient returned healthier for another IVF attempt. This time, she successfully conceived and later delivered a healthy baby. The journey from a life-threatening infection to parenthood within a year underscores both the unpredictability of reproductive health and the resilience of patients.

The case also offers important clinical lessons. Endometriosis-related “chocolate cysts” can harbor infection, and even routine egg retrieval may rarely trigger abscess formation. Persistent fever, escalating pain, or poor response to antibiotics after IVF should prompt immediate medical evaluation.
