Dr. Kamath, I completely agree with you, if I would get chance to evaluate the existing their safety documents sure I can prove that there is a big gap to maintain the safety at work site
Normally oil & gas industry we gather all pepper work prior to start the job which is piping & instrument diagram (P&ID) emergency rescue plan, emergency evacuation plan, Confined space entry plan (if needed) risk assessment, gas level test for toxic , flammable & LEL proper communication, use of personal protective equipment etc, etc which can save the near misses, accidents, property damages and fatality

How much have they paid you? The company is not the one to investigate. The insurance company and independent agencies have to conduct the investigation to determine the cause. In any case the management is liable. We need Mamata here to throw the lot of them in jail and lose the key.

Problem with Indian Industries is Chalta hai ( nadee tade -in kannada) attitude .In science of industrial Safety this attitude is called INDIFFERENCE is one of the main behavioural hazard .Contract employees are generally not well trained .Solution : MRPL should conduct (I dont know whether they are conducting or not )1.Mandatory safety briefing upon every new worker introduced 2. Conduct weekly safety meetings department wise 3.Proper planning for each job to be done 4.Continuous observation process to identify possible safety hazards and their immediate correction .All American and British Companies follow these guidelines mentioned above and it is joy to work with them as you will be knowing how to work safely

It is unfortunate that such incidents happen in Public sector company like MRPL. This shows what safety standards we follow in Indian industry falling under government of India and not to think of any private company. This is because there is no value to human life in India, which can be understood by the way Bhopal tragedy is still being handled. There is no severe punishment to defaulters and if at all it is there, corrupt officials allow the culprits to escape from the blame. The poor person having no connection to the politicians or govt. officials die like this and his/her family does not get justice properly, forget about the compensation due to him/her. Unless we ourselves get edjucated and start asking questions to govt officials and fight for justice, without fearing local 'goondas', we will not be able to improve our life. We need to fight corruption united all the way like Anna Hazare and see that all govt. officials and all wealthy & mighty politicans and goondas are behaved properly, repecting law and order of the country. This will only stop such incidents in future in our country. I sympathies with the family of persons got injured in the incidents but fully hold MRPL staff responsible for the incident and demand their immediade removal from service.

In reality most of the workers wear slippers and climb heights without safety belts. May the departed soul R.I.P. & speedy recovery to the injured.

What is the use of investigation's
and committe's detailed report, can this report prevent a further accident, the answer is never for no one is bothered about safety and this is a true fact. The person dead is dead and gone nothing can be done and no further action will be taken against the safety of people. The contractor's are there to make some money not to spend on their employes safety, which will reduce their profits. Safety of the employees will be looked after when there is a very settlement paid by the employer to the the employee in case of accidents, it should be so high that the employer should think a thousand times about the safety of the employee, then I will gaurentee you there will be minimum accident's.Minimum compensation for losing a life should be 1 crore even minor accidents should be treated seriously. The Supervisor/Foreman should also held responsible for negligence if any. May the departed soul rest in peace, may God give strength to the family to bear the loss.

Occupational Health and Safety Advisory Services (OSHAS) training must be made mandatory for all employees (permanent & Contract). This will bring down the accident and incident within our factories.

Anyway acidents are common incidents in oil industry. But here in MRPL it's seems to be very often.Just some days back there was an insident.I think safety section and MRPL management is not doing enough to control these incidents. At least try to creat a fearless environment to work freely for it's employees.

NO DOUBT, MRPL will be maintaining high safety standards. However, accidents usually occur, due to human error. 'Prevention is bette than cure", so says the old saying.

That the company is investigating the incident, so let us wait n see.

We are into the business of safety audits & conducting safety workshops. In our experience, we see sometimes, and most of the time, human errors cost terrible loss.

Only if, the user has adhered to safety norms, a lot could have been saved. Of'course, one can blame the machine for certain calamity.

My sympathies to the family who has lost their dear one, and also pray for speedy recover to those in the hospital.

    Tkx DaijiWorld for your reporting.
