Udupi: Woman given blood of wrong group, nurses' negligence alleged

Daijiworld Media Network - Udupi (HB)

Udupi, Oct 2: In a case of alleged negligence by the district government hospital, a woman patient developed complications after being given blood of the wrong group.

Sumitra Poojary (33), wife of Raghavendra Poojary from Hejamady near Padubidri got admitted to the government hospital for women and children in Udupi on September 27 for delivery of their second child. She gave birth to a healthy baby the next evening.

On September 28, her doctor said that Sumitra needed blood (O-positive), and accordingly Raghavendra brought the required blood units from the Ajjarkad government hospital blood bank, without verification as he did not think the blood bank would make a mistake.

Thereafter, three nurses began transfusing the blood into Sumitra, at a time when Raghavendra was not in the hospital.

After transfusion of two-and-a-half units of blood, Sumitra developed discomfort in breathing and acute body pain. The transfusion was immediately stopped. Her hands began to swell, and as her condition worsened, the nurses, suspecting infection in the blood, asked Raghavendra to return the remaining blood and get another unit.

Growing suspicious, Raghavendra narrated the incident to the blood bank officials, only to realize that he had been given the wrong blood group. He alleged that the nurses had mentioned the wrong group in their note to the blood bank. He immediately gave a written complaint to the district health officer (DHO).

Thereafter, district surgeon Mahendra examined Sumitra and prescribed some medicines which improved her condition. He also apologized to the couple and assured that such blunders would not happen again.

Raghavendra alleged that nurses in the hospital treat outpatients with a degree of indifference. "Not even a single duty doctor came to check my wife's condition," he alleged, adding that the nurses who transfused the blood had also verbally abused him.

Damayanti, who was the chief physician of the hospital was transferred Brahmavar hospital a month ago, and Kishori from Lady Goschen Hospital, Mangaluru, replaced her around 15 days ago.

Sources said that the childcare unit at the hospital has no sufficient consultants and there is a shortage of medicines too. When required, the medicines are bought at private medical stores, sources said.

Raghavendra is now planning to approach health minister U T Khader for justice.

He also alleged that one of the nurses told him that family planning procedure was compulsory at the hospital after two children, and that if the couple does not agree to it, they would not be allowed in the hospital for the third delivery.

He urged that the health minister take immediate action and conduct a detailed investigation into the functioning of the hospital and the nursing staff.

Nurses take care of patients 24 hours a day continuously while doctors see the patients for a few minutes in a day. Knee jerk blaming the nurses indicates how our society is reaching towards degeneration.
An unbiased root-cause analysis by a third party is needed and the negligent party needs counseling. Yes- counseling and not the punishment. Punishment is reserved only for a person who does wrong willfully and wantonly. It is worth exploring the duty hours, tiredness and commitment of the nurses involved.
New policies should be brought in place so that these errors will not happen in the future.
Sensationalizing this news by newspapers at this time is wrong.

    Hi Guys

Very interesting case brought to our concern by our beloved writer.Thank you for your services man!!

First speaking about blood transfusion 2 in 100 patients show serious side effects which needs medical attention. These symptoms include anxiety, chest pain, pain at infusion site, respiratory distress, Loin/back pain and headache.
Side effects in blood transfusion is common issue.

These side effects caused by recipient antibodies reacting with white cell antigens or white cell fragments in the blood product or due to cytokines which accumulate in the blood during storage.

I did speak and talk to people related to this case and I found that there was no evidence of mismatching whatsoever. The patient believed the side effects to be mismatching and I repeat again no solid prove of mismatching. The side effects observed by the concerned people were not due to mismatching, and thanks to alertness and knowledge of nurses, immediately blood transfusion was stopped and patient was treated. Thanks to them☺☺☺

Now its the time to BLAME SOMEONE
"All things that go bad in government hospital are due to negligence of nurses" COME ON GUYS STOP BLAMING NURSES -seriously

Nurses take care of patients round the clock. Its due to their work a hospital can exist. When compared to doctors, nurses take care of patients and help the patient to be healthy and happy. So a special thanks to our Florence Nightingales.

But we need to blame someone that's in our blood LOL.

A very good effort by news writer to write this article with just one sided argument.

BELOVED WRITER Thanks man you wasted my 30 mins in writing this comment and referring articles.

First transfusion had no problem patient was fine during second transfusion the reaction occurs that means either the blood bag must be wrongly labelled, not properly matched..as per the news report...wrongly labelles and matching not done properly then blaming nurse of the ward is wrong...blood bank has to take responsibility...

Mr Albert, get some medical knowledge, contest in the election, become health minister and then see how is it to deal with people. Simply blaming you will not understand the responsibility. He is one of the best ministers of karnataka all time produced.

    In India
If you get a government job!
    Either as watchman
    Or as Chairman!
    Doctor, health officer
    Or Chaprasi!

You will be like a governor
No one can talk to you, no one can touch you,no one can take any action until your retirement.
I had an experience to visit a poor lady in a government hospital every visit we had to pay money to watchmen to allow us inside.
You can imagine even watchmen can make money from patients and their visitors.

@A.Gladys Monteiro,Bharain,do you know what are you writing?without proper nowledge please dont comment. It was a mistake on the whole.Should dismiss all People who are involved.First the blood bank who gave the blood, second the one who adminstered the blood without varifying.Usually blood should be given to the Patient only after the physician/doctor from ward signed or given his o.kwhen he has inspected .It is a wonder Patient is living still. Many times Patient develope reaction immediately and DIE.may be blood in this case was not o Group .Her husband waas not at fault.since her groupe was o thats may be saved her.and about Family planning,it is None of their nurses Business to tell when she Comes third time not to accept.Afterall they can advice them but when she Comes should take her and give Treatment. After this mischievous I advice her go to another Hospital for any Treatment.what a gluck she /Family had.cant imagine.dismiss all who gave and staff at Hospital.dont want another Family to have Problem.Ask Minister about this Treatment and file a case .dont leave it so.

Read the book Named CHECKLIST MANIFESTO by Dr Atul Gawande a US based doctor. You will just realize why docs or med professionals do these mistakes......it happens and will keep happening until a disciplined method of checking and confirmation is adopted by docs and hospitals
