Mangaluru: Now, fast recovery technique for joint replacement surgery at KMC

Hip and knee joint replacement surgeries have universally been accepted as the best of all known surgeries in terms of changing the quality of life by quality adjusted life year (QALY) measurements as certified by WHO. Hip and knee joint replacement surgeries involve resurfacing of worn out bony surfaces of a joint so as to enable smooth movements. The cut bony surfaces are substituted with prostheses made of metal alloys and tough plastic or ceramic inserts.

Careful examination and a detailed discussion of the patients’ individual problems helps the surgeon select the right type of operation or use one of the new procedures to prevent joint replacement in milder cases of arthritis. Secondly, evidence from the past has shown that any joint surgery will be less successful if there is delay in movement of the joint following operation.

‘Enhanced Recovery’ as the technique is known was originally developed in the western countries as a mechanism to increase turnover for surgeries in high- volume joint replacement centres. The techniques essentially include modifications in anaesthesia and surgical tissue handling so as to give better pain relief and less drowsiness after surgery.

Endorsing this technique is Sheela who underwent a total knee replacement on February 1, 2015. She was able to bend her knee and commence exercises soon as she was awake in the recovery room of theatres. She was assisted to get up 5 hours later and walked to the toilet with a frame for support, while putting full weight on her operated knee. She was also able to climb stairs 2 days after the operation and was discharged on the 4th day. By then she was comfortably able to raise her leg, get up and walk without anyone else’s help and bend the knee beyond 90 degrees.

“When the patients walk on the day of surgery, their confidence rises immensely. Subsequent progress in recovery and function is much quickened. Patients even manage stairs in a couple of days. Both hospital stay and complications like infection and blood clotting in calves (venous thrombosis) that used to be common following joint replacement are much reduced. As joint replacement procedures are planned, we have the opportunity to optimise the patient’s status before the surgery. As total hospital stay can be reduced to five days or less, cost to the patient reduces as well. This is certainly a win – win from all angles,” said Dr Yogeesh D Kamat, consultant knee and hip surgeon – joint replacement and sports injuries, KMC Hospital.

With a personal experience of over 2000 joint surgeries, Dr Kamat was introduced to enhanced recovery protocol four years ago during his fellowship in England. Where he used this technique over the last two years as a consultant in London for about 250 subsequent cases, and is practicing the same in Mangaluru, with comparable early results. He also undertakes pre- and post- surgical scoring of all surgeries, so that the outcome can be objectively benchmarked against published international standards

With the increasing scale of joint replacement operations across our country, it is imperative that dedicated units offering such services incorporate the best standards of care and rehabilitation. Every patient must be able to walk on the day of surgery following a straightforward joint replacement, unless they are having a complex procedure or are medically compromised.
