At this point, 6:05:00 am on May 22, 2010, the Cockpit Voice Recorder (CVR) went blank. The Captain, soon after touchdown, attempted to take off again against air safety norms and failed. Few minutes later, Air India Express IX 812 overshot the tabletop runway at Mangalore’s Bajpe airport, its right wing hit a concrete structure supporting the antennae, crashed into the boundary fence and fell into the gorge below.


Within seconds, the flight from Dubai was a ball of fire, claiming 158 lives on board. Eight passengers survived.

The final investigation report has held the Captain’s failure to execute a safe landing as ‘the direct cause of accident’. The report, which has been accepted by the government, said Captain Zlatko Glusica, who was in command during take-off from Dubai and landing at Mangalore, carried on with the “unstabilised approach” during landing and ignored both the first officer’s (HS Ahluwalia) three calls to ‘go around’ and several warnings to ‘PULL UP’ and ‘SINK RATE’ from the Enhanced Ground Proximity Warning System (EGPWS).

Being at an altitude much higher than mandatory, the aircraft lost considerable runway length during descent, left with little to brake. EGPWS alerts the pilots in case the aircraft is in danger of flying into the ground or an obstacle. Unstabilised approach means the aircraft did not follow the prescribed speed, descent rate or vertical/lateral flight path parameters at the time of landing.

Despite adequate rest period prior to the flight for him and the co-pilot, the Captain was found sleeping for the first one hour and forty minutes out of the total two-hour and five minutes recorded on the Cockpit Voice Recorder (CVR), the report said. “As a result of relatively short period of time between his awakening and the approach, it possibly led to impaired judgement,” the report said.

The “sleep inertia” coincided with another phenomenon, often said to be a nightmare for pilots, Window of Circadian Low (WOCL) which refers to the period between 2 am and 5.59 am, when body temperature, alertness and ability to be awake is lowest. The report observed that a mandatory pre-flight medical check was not conducted for the flight crew before they departed from Mangalore. However, the report ruled out intoxication or self-administered drugs by the flight crew.

    Its sad that this accident happened...Its prudent we should learn lessons from this and see that these types of accidents dont happen in future....

    Its not enough give rest to pilots... they should be secluded to prevent distractions(???).....and lack of sleep....

    The entire fault lies with Mangalore International Airport which by no stretch of imagination is an International Airport as the length of the runway is pathetic. There is absolutely no scope for mistakes.

    They found at least 15 fake Pilots in Air India crew. Wake up DGCA wake up don’t play with life of people. We are travelling by Air India to support our country though, Air-India Flights are delayed, cancelled, derailed we know death is awaiting us. Mangalore Airport authority’s plz customs harass us. In last Dec my friend travelling with me bought 4 cigarette cartons, customs said it’s not allowed, then we divided it into two still officer said only one allowed. I told him my dear I am not smoking, 1st time in 19 yrs I bought this stuff and was not aware just one carton is allowed. I am taking it for my Father-in-law who is sick, so that he may smoke it n die soon. He laughed loudly and allowed us to go.

    For the families of those who lost their near and dear one's the pain is not over.some of the bodies of crew members and and passengers were not identified and burried in mass grave.most heartening 24 children died, It was the worst disaster in aviation history and first for mangalore, but what lessons were learnt? the promissed lengthening of runway by 1000 feet not yet started after one year and extension of safe area arround the runway, now again the rainy season started and same as before flights will be diverted to calicut every alternate day.Blaming just the pilot will not absolve us of our responsibility.At least our local leaders support airport director Mr. Vasudeva - an able person- in completing the herculan task

    As per cockpit voice recorder that recored snoring of the pilot who was in deep sleep. Pilot was asleep for much of the three hour flight. One of my friend pilot say Captain Glusica who approached the runway at the wrong angle and ignored warning signs was to blame for crash. Most serious aircraft accidents are primarely the result of human error usually the pilot. One survey shows 1 in 5 piots face asleep during the fights. Even pilot and co-pilot facing asleep at same time. The flight was kept on course by the plane's Auto Pilot system. Now days in India fake flying licence and fake pilots..!! So far 13 fake pilots have been arrested. All Indian pilot licences must examine and found any guilty must terminated for permanent. In India nobody dont know what is original or fake. Our ex civil aviation minister Mr.Praful Patel should be investigated because this fake pilot scam ballooned during his term as a civil aviation minister. Our new civil aviation minister Mr. Vayalar Ravi must wakeup at least now and clear the mess.

    Many fake pilots are flying aircraft in India and there is the recent scam of fake pilot getting licence thru political power contacts. Praful Patel has done a bad job in his tenure only pumping money to Air India to survive.

    Could have avoided... but too late. Hope Mangalore incident will be the last and first in the History. Many things we/ govt had learned i.e forged people travel fake passports, sleepy pilots, Quick firefighters?, extended runway, modern airport facility, thieves, money lovers, and off course departed souls. Many lives got changed for many reasons.
