Modi government puts brakes on India's universal health plan


New Delhi, Mar 27 (Reuters): Prime Minister Narendra Modi has asked for a drastic cutback of an ambitious health care plan after cost estimates came in at $18.5 billion over five years, several government sources said, delaying a promise made in his election manifesto.

Modi has had to make difficult choices to boost economic growth - his government's first full annual budget, announced last month, ramped up infrastructure spending, leaving less federal funding immediately available for social sectors.

The health ministry developed a draft policy on universal health care in coordination with the prime minister's office last year. The National Health Assurance Mission aims to provide free drugs, diagnostic services and insurance for serious ailments for India's 1.2 billion people.

The health ministry proposed rolling out the system from April 2015, and in October projected its cost as $25.5 billion over four years. By the time the project was presented to Modi in January the costs had been pared to 1.16 trillion rupees ($18.5 billion) over five years.

That was still too much. The programme was not approved, three health ministry officials and two other government sources told Reuters. Three officials said the health ministry has been asked to revamp the policy, but work is yet to start.

"The constraint on India's financial resources was conveyed to health officials, and even to those from other ministries," said one government official who is not from the health ministry but attended the meeting where Modi was present.

The meeting was held in January and the discussions were not made public. All of the sources declined to be named because of the sensitivity of the discussions.

Officials at the prime minister's office and the finance ministry, as well as the health ministry, did not respond to requests seeking comment.

Modi's manifesto ahead of the election that brought him to power last year accorded "high priority" to the health sector and promised a universal health assurance plan. The manifesto said previous public health schemes, that have been mired in payment delays recently, had failed to meet the growing medical needs of public.

Modi has another four years left in his first term to fulfil the promise.

India currently spends about 1 percent of its gross domestic product (GDP) on public health, but the badly-managed public health system means funds are not fully utilised. A health ministry vision document in December proposed raising spending to 2.5 percent of GDP but did not specify a time period.

So health experts were dismayed when the federal budget for the full-year starting April raised the allocation for the country's main health department only by about 2 percent from the previous year, less than inflation. The meagre increase dimmed prospects for the massive health plan, they said.

"How can it happen when you have truncated resources?" one health ministry official asked.


Rework, Disappointment

The health plan was drafted in consultation with Modi's office and an expert panel, including an expert from the World Bank. The proposal included insurance to cover more expensive and serious ailments such as heart surgeries or organ failure.

Two government officials said this benefit will be withdrawn. The health ministry was also asked why the new plan could not be simply combined with existing health schemes.

The decision to slash the level of insurance coverage will have an impact on India's thriving private health sector, which would have seen huge business growth as the programme channelled millions of new patients into private hospitals.

Currently only 17 percent of India's population has some form of health insurance.

Anjan Bose of NATHEALTH, a group that represents such private firms, said the delays in the programme would disappoint both the industry and the public.

"Greater availability of funds for public health care under this programme would have further boosted growth of the private medical industry," Bose said.

    ... to everyone which a true leader should not have done. You do not get votes with that kind of promise which you yourself know which is not achievable when you promise. Now other promises getting hit as well because just like that Desai and Charan Singh, he do not have the big picture of the country and he thought the India's size is equal to Gujarat's and only Gujarathi's live in India. When you're projected as a prime ministerial candidate, you should know what is achievable and promise those things only. Now if you are telling no other government in the past fulfilled it's promises, then WHAT difference this government has from the previous governments?? You give the same respect to Modi that you gave to MMS, Deve Gowda, IKG, VP Singh, PVN, Rajiv, Indira, Charan Singh, Shastri, ABVP and Nehru. WHY do you TREAT Modi differently when he has not done anything so far nor fulfilled his election promises??

    I respect Mr. Modi as the Prime Minister of India.

    Even while evaluating the accomplishment of Mr. Modi in Gujarat in this three years tenure as the CM of Gujarat, certain visible things happened like the highways and the cities had some better look.

    But the villagers were pushed back both in education and health care, Gujarat was far behind many other States, ranking in the 10th position in overall status.

    Through some high level political manipulation, he was landed in New Delhi to rule a nation of highly complex problems-religions-cultures-varying level of literacy and standard of living.

    The electoral promises he gave was worth trillions of dollars. How did he promise likewise? He was simply speaking what he felt to make the electorate happy and to cast the vote! The electorate thought that he has some supernatural magical power to fulfill everything as promised, like one guru said in Mumbai years back that he will walk over the water and charged a very high entrance free but went downwards faster than Peter's experimental walking towards JESUS.

    Sridar Rao, Very good analysis but it is not impartial though. You asked what is the achievement of congress over 60 years. Are you a proud Indian? if so for what?? Is it India have it's own space programme when countries like UK, France and Germany don't have. Is it because India successfully completed Mangalyaan?? Is it because India produces its own defense missiles, ships and even started building submarines, built light combat aircraft and exports arms to many counties. Is it because we can proudly say we are a nuclear power? Is it because we are the software center of the world? For your information, AIIMS is a world class medical felicity which is still government owned. In addition to this there are so many world class medical/engineering colleges, business colleges which is in India. Do you think India had the same status in the world, had same GDP, had same say in international community, had same economy that we have today in 1947?? Don't you think even with all the corruption Congress did some thing. You have to ask another question I guess, we did had non congress governments in the center in between that 60 years and what is their achievement? Was corruption eliminated during Desai regime or Charan Sigh's? Did corruption vanished during VP Singh, I K Gujral, Deve Gowda or ABVP's time? The answer is a big NO. It will take another 250 years to make India corruption free. Do not think it will happen overnight? I am not supporting any arty here. I did voted to V P sigh's regime , I did voted to ABVP's regime as well as to Rajiv, PVN and MMS's regime and recently to Modi's regime. What strikes strange for me is even in Karnataka we brought BJP for a change and you know corruption completely disappeared. The problem with BJP is that it projected itself as clean party (against the corruption and it is a party of discipline with no dissent) and when they came to power, we saw different story in both fronts. Modi promised to give few lakh rupees free...

    This Government is now advancing towards some very serious traps.

    Cutting down on the healthcare will be affecting the lowest stratum of the society which means, for the BJP, poor human beings have no value. Mr. Modi will be spending more money for his expensive changing of outfit every day by the lakhs.

    Taking off the food from the dinner table through religious suppression thus making millions of people hungry and millions jobless. The beef ban news is already in many news papers like the New York Times. Already the number of tourists are declining to India due to the economic recession mainly in the European countries coupled with the bad news of rapes. The beef ban will be adding more troubles to the tourists industry.

    Minority attacks. The culprits caught in 48 hours in Mumbai will be turning as big secret drama later on.

    Wait for many other drams to take place in India.

    Whether any government so far ruled the country has fulfilled the promises they had given to public in the election manifesto. Health of the nation is important. That does not that you have go for a plan with out sufficient funds for the plan and collection of revenues. How many citizens have participated in Modies SWacch Bharath Andolan. Please touch your heart and tell. It is easy to comment against any ruling party. What is the contribution of the congress which ruled the country for more than 6 decades. Ultimately every thing should not end up with spending of budget without any result. Each man gets some clue some where to reduce the cost factor. Instead of commenting adversely we shall try to analyse why this decision has taken place. Whether it is only due to shortage of funds or for any reasons. Each rupees spent should give the result expected. We will wait and see wihtout going for post haste comments.

    You are very correct. Mr. Modi's right hand people systematically tried to erase the Christmas from the Indian calender through replacing CHRIST's birth to Vajpayee's birthday along with another trick of "good governance"
    day. Schools were opened on the Christmas day, but the plan didn't work.

    The electorate who did vote for Mr. Modi had no idea about the cost in trillions for all the Himalayan economic promises to change India into an economic paradise on earth.

    Putting dry leaves in a concrete foot path in New Delhi and cleaning the dry leaves became headlines in the revolutionary " India cleaning mandate". What happened to the river cleaning?

    Now, the Government of India doesn't have money to take care of the health of the poor people as promised through the universal healthcare.

    Thus the beginning of the great accomplishment of the BJP Government before the 2015 Christmas, we are going to witness the fulfillment of multitude of electoral promises. Mr. Modi will be traveling to more countries in high glamour to impress the the electorate of India-keeping his mouth shut while the minorities are under bewilderment.

    It's easy to give fake promises.But in real it's impossible to implement..One should learn the Economy well before promising something impossible (just to fetch votes)..

    Congress has scr**ed up the public health sector so badly that ordinary citizens like you & me dispel from going to Govt hospitals for any kind of health issues...rather we visit private hospitals..In Govt hospitals, pathetic service, no doctors, no nurses, no beds, no emergency services so who will go there?

    This system which has been subject to abject corruption & looting in the name of poor will take a long time to come back to normalcy...

    Dear J Anata, have you ever visited govt hospitals like Vani vilas, Victoria and KIMS in banglaore? Visit Vani vilas hospital you will see most of the complicated pregnancy cases which private hospitals refuse to take gets operated successfully only in Vani Vilas hospital.

    Around 15 years back there were no private hospitals and only govt hospitals were there. The wenlock and Lady Goshan hospitals were best in Mangalore. We had lot of private practitioners, but still Govt hospitals were useful for treatment. The privatisation of Medical education resulted in opening up of private hospitals and corporate hospitals, and govt started stopping funding govt hospital .There was a nexus between Doctors-Private Hospitals- Politicians. As the politicians become owners of the educational institutions, the govt hospitals started neglected. Apart from this private institutions started using the Govt hospitals as study centers and the importance of care of patients came down. For Govt it became a business to lease Govt hospitals to private and earn money. For private institutions it became a way of making money ,even without having their own building of hospitals for teaching. The poor state of Govt hospitals lead people migrate to private. But still there are good govt hospitals in rural areas where people depend up them.
